How to Write a Winning Claim Appeal Letter (With Templates)

When to Appeal a Denied Claim (and When to Let It Go)
The quick rule: if the denial reason is reversible, appeal it. That includes medical necessity disputes, missing modifiers, eligibility errors, timely filing denials where you have proof of original submission, and missing or misapplied prior authorization.
When should you let it go? When the service is genuinely excluded from the plan with no parity or policy argument available. In behavioral health, that's rarer than you'd think.
Check the EOB the day it arrives. Internal appeal windows range from 30 to 180 days depending on the payer, and the clock starts on the denial date.[2] Missing the window is the easiest way to lose a claim you could've won.
A behavioral health-specific note: medical necessity and prior authorization denials are almost always worth appealing. Federal parity laws and published clinical guidelines tend to support you.[3]

Anatomy of a Winning Claim Appeal Letter
A good appeal letter is short, structured, and easy for a reviewer to process. Here's what every claim appeal letter needs.
Header block: practice name, NPI, tax ID, address, phone. Then patient name, DOB, member ID. Then claim number, date of service, total billed.
Subject line: something the reviewer can find in two seconds. "Formal Appeal, Claim [Number], DOS [Date], Patient [Initials]" works well.
Opening paragraph: state plainly that you're formally appealing. Reference the denial date and the specific denial reason or code on the EOB.
Body: address the denial reason directly with evidence. Quote plan language. Cite clinical documentation. Reference parity protections when they apply. Don't argue from opinion. Argue from policy.
Closing: state what you're asking for (reprocess and pay the claim) and list enclosures so the reviewer knows what's in the packet.
Tone matters as much as content. Reviewers approve appeals that read like a clean argument. They set aside appeals that read like complaints. Compare these openings:
Weak: "We strongly disagree with this unfair decision and feel the denial was wrong."
Strong: "We are formally appealing the denial dated [date] for claim [number] on the grounds that the service met the plan's published medical necessity criteria."
The second one tells the reviewer exactly what's being challenged and on what basis. That's the whole job of the opening.
Five Insurance Appeal Letter Template Examples by Denial Type

Template 1: Medical necessity denial
We are formally appealing the denial dated [date] for claim [number], date of service [DOS]. The plan denied [CPT code, e.g., 90837] as not medically necessary. The rendering provider's letter of medical necessity (enclosed) documents the patient's clinical presentation, treatment plan, and the rationale for [60-minute psychotherapy / specific service]. The service meets the plan's published medical necessity criteria for behavioral health. We also note that the federal Mental Health Parity and Addiction Equity Act requires the plan to apply medical necessity standards no more restrictively to behavioral health than to medical/surgical benefits. We request the denial be overturned and the claim reprocessed.
Template 2: Timely filing denial
We are formally appealing the denial dated [date] for claim [number], date of service [DOS], on the grounds of timely filing. Enclosed is the clearinghouse acceptance report dated [date] confirming the original claim was submitted within the plan's filing window. Please reprocess the claim and apply payment.
Template 3: Lack of prior authorization
We are formally appealing the denial dated [date] for claim [number]. The denial cites lack of prior authorization. Authorization [number] was on file at the time of service for dates [start–end] (enclosed). [Or: Per the plan's behavioral health benefit summary (enclosed), prior authorization is not required for [service / place of service].] We request the denial be overturned and the claim reprocessed.
Template 4: Non-covered service or out-of-network
We are formally appealing the denial dated [date] for claim [number]. The denial states the service is non-covered. The plan's behavioral health benefit summary (enclosed) lists [service / CPT] as a covered benefit under [plan section]. Federal parity law requires behavioral health benefits to be applied at parity with medical/surgical benefits. We request the claim be reprocessed and paid.
Template 5: Coordination of benefits or duplicate claim
We are formally appealing the denial dated [date] for claim [number]. The denial cites coordination of benefits. The primary payer EOB is enclosed, showing [paid amount / patient responsibility]. As secondary, your plan's COB determination should apply to the remaining balance. Please reprocess the claim and apply payment.
These are starting points, not finished products. Every appeal needs the specific clinical and policy detail of the actual case attached to it.
Documentation That Turns a "Maybe" Into a "Yes"
The letter is the wrapper. The documentation is the case.
For most appeals, build a packet with:
- Letter of medical necessity from the rendering provider
- Treatment plan and relevant progress notes (PHI-minimized where appropriate)
- Original CMS-1500 and the EOB showing the denial
- Proof of timely filing, if relevant (clearinghouse receipt, fax confirmation, certified mail receipt)
- Prior authorization confirmation, if applicable
- Direct quotes from plan language that support your position
For mental health denials, cite the Mental Health Parity and Addiction Equity Act whenever the denial appears to apply stricter standards than what the plan applies to medical or surgical services.[3] Parity is one of the strongest tools therapists have, and most generic guides skip it entirely.
Organize the packet for the reviewer's eyes, not yours. Cover letter first. EOB second. Then medical records. Then plan language citations. Number the pages and reference them in the letter ("see attached page 4"). A focused 4 to 6 page packet beats a 40-page dump every time.
The frequency question matters in behavioral health, too. If you bill weekly 90837 and the payer pushes back, the letter of medical necessity should explain why a 60-minute session is clinically required given the patient's complexity, not a default.
How to Submit, Track, and Follow Up
Submit via the method the payer specifies. Use the portal when available, fax when required, certified mail with return receipt for paper submissions. Always keep proof of submission: portal screenshot, fax confirmation, or certified mail receipt.
Calendar a 30-day follow-up the moment you submit. Non-urgent internal appeals are typically decided within 30 to 60 days. Urgent appeals (think safety-related continuity of care) must be decided within 72 hours.[2]
Every follow-up call: log the date, time, rep name, reference number, and the substance of the call. Those notes become evidence if you need to escalate to an external review or file a complaint with the state.
If the internal appeal is denied, you typically have external review rights. An independent reviewer outside the plan looks at the case, and their decision is binding on the payer.[2] The denial letter must explain how to request it.
One more thing worth doing: track every denial and appeal by payer and reason code. Three appeals on the same reason against the same payer isn't a coincidence. It's a payer policy issue, and a parity complaint to your state Department of Insurance may fix the pattern faster than appealing each claim individually. Final Thoughts: A Routine Process, Not a Crisis
